Claim Forms
An ISS Claim for Payment must be submitted at the end of every quarter accompanied by a Attendance Record which has been signed by the Additional Worker/Carer on a daily basis .
New, streamlined Claim for Payment forms have been developed to accompany the IPSP Guidelines 2009-12. Please refer to the Guide for instructions on how to complete these forms.

Old version Attendance Records and Claim for Payment forms will be accepted until January 2010. Please use the new forms to claim for all periods from Jan-Mar 2010 onwards.
